I tried using the online translators but got a couple of different translations, can you help me out please?

I make these banners for a guy as a donation and now he wants them in spanish.

The wording is

Are you going to Heaven?
Take a two question test to find out.
Free Test

Here is the translation I have...

¿Va usted al cielo?
tome una dos prueba de la pregunta para averiguar

¿Va usted al cielo?
lleve una prueba de dos preguntas descubren

I wouldn't use either one.

The mind set here is to think in spanish rather than in English. I learned spanish while working in a spanish speaking country over a number of years. I speak it evey day with my wife. But I can't turn off the fact that English is my first language, I have to think about it. Your computer translation is way off. One has the 2nd statement backwards and the question is unclear as to if it referrs to heaven, the sky, or in some cases the ceiling of a house.

Felix would be better for this, but here goes.

Se va usted al cielo con Dios?
(Make an extra question mark and place it upside down in front of the sentence.) And why not mention God's name, that's what this is about, right?

Toma usted una pueba de dos preguntas para saber.

I would go with what Santo said, and at the bottom put La prueba es gratis (free test)
