For those of you who offer website design to your customers.....how do you work out the pricing for the domain name? Do you set up and then give info to customer to make sure they follow the yearly cost of the domain name....or do you look after it and bill your customer each year?
Suggestions are welcome on how to handle this.

Easy.

Take care of the initial registration process for them and bill for it, but enter THEIR info into the registration form so when it's time to renew, THEY get the bill and have to take care of it. Also let them know the exact name of the registration company because they will receive letters from other registrars saying how their domain will expire soon and they should renew, yet these companies don't make it clear they're not the actual registrar they used.

This is how I handle setting them up with a hosting company too.

I only want the design and technical work, let someone else handle the recurring billing. It's enough work to keep track of my own multiple domains and hosting accounts, let alone everyone else's too. Add on top of that site updates and maintenance, it piles up quick.
